We are a Medicare Accountable Care Organization designed for Federally Qualified Health Centers in Florida.

Our Members

Healthcare organizations are built to make the complex health system easier for patients to understand by putting them at the center of their care.

Accountable Care Organizations (ACOs) are responsible for the overall quality, expenses, and patient experience related to care provided to their assigned fee-for-service Medicare members.

The Florida Association of Community Health Centers, Inc. (FACHC) is a private, not-for-profit membership association committed to building the capacity of Florida’s Community Health Centers in leading the primary care safety net, ensuring equitable access to quality care for all Floridians.

In addition, FACHC collaborates with strategic partners to assist Community Health Centers and is a bridge for health centers across all policy and advocacy activities, giving a voice to health centers and primary care at the local, state, and federal levels.

Our Members

Federally Qualified Health Centers help increase access to crucial primary care by reducing barriers such as cost, lack of insurance, distance, and language for their patients. In doing so, they provide substantial benefits to their community’s health care system.
